Written Exam Site Application

This application consists of three parts: Site Information, Exam Date Availability and Costs, and Written Exam Site Security Agreement.

All fields on the form must be completed. Incomplete forms may delay authorization. You will be notified by email when your exam site has been authorized. Allow at least two (2) weeks for your exam site to be authorized and activated on the CIC registration system.
